To apply for a credit account, follow the "Credit Application Instructions." The Credit Application Packet contains the following documents which are available in the Acrobat Reader format (PDF files).

Once the documents are filled out completely, sign them where appropriate and follow the "Credit Application Instructions." If you have any questions, contact Sanitation Districts' Credit and Collection Specialist, Nancy Hayes, at (562) 908-4288, extension 1120.

All four forms must be completed to assure prompt approval of your application.
